Medical Emergencies
In the event of a life-threatening medical emergency, call emergency medical services at 911 immediately. DO NOT WAIT! Inform the emergency room staff that you are a patient of Trinity Integrated Medicine and they will contact us.
After Hours: Urgent Medical Needs
If you have an urgent medical need and need to talk to our doctors after our regular hours, please call our office at (623) 873-0112. We have a 24/7 answering service and your call will be returned promptly.
Insurance
At Trinity Integrated Medicine, we accept Medicare, Medicaid, and most major insurance plans. Please contact your insurance company or you may call our office at (623) 873-0112 to find out if we accept your plan.
If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience we accept cash, checks, Visa, MasterCard, Discover, American Express, and CareCredit.

Referrals
If Dr. Rodd Aking, Primary Care Physician suggests a specialist evaluation or diagnostic testing that requires a referral, we will make every effort to provide this for you as soon as possible.
Some referrals require insurance preauthorization and cannot be processed immediately. Insurance guidelines prohibit us from backdating referrals. Prescription Requests
Prescriptions and refills for established patients of Trinity Integrated Medicine are typically issued during our regular business hours (see above).If you require a refill, please call your pharmacy and request it at least 5 days before your medication runs out. The pharmacy will then fax a refill request to us, and we will process it accordingly.
Refills are not issued during evenings, weekends, or holidays, as doctors do not have access to your medical records during those times. It is helpful to anticipate your refill needs in advance to avoid any interruptions in your medication treatment.
Office Policies
This looks like a comprehensive patient policy guide for Trinity Integrated Medicine. Since the original text was a mix of both languages, I have organized everything into two clean, separate versions for you.
Appointments
Appointments can be booked in person, over the phone by calling (623) 873-0112, or requested online. When booking, please state the reason for your visit so we can allocate adequate time. If you require a complete physical or specific tests, let us know in advance.
- Walk-ins: Walk-in patients are accepted.
- Cancellations: Please provide 24 hours' notice. Missed appointments will incur a charge that must be paid before your next visit. More than two consecutive no-shows or cancellations may result in the discontinuation of services.
- SMS Consent: You will receive a printed copy of your consent to review. By consenting, you agree to receive text messages for appointment confirmations or important communications if you cannot be reached by phone. You can opt-out by texting "STOP" or get help by texting "HELP." Message and data rates apply.
Test and Lab Results
Patients are expected to return to the office to receive test results; your doctor will advise you on a return date during your visit. While the clinic may call you when results are ready, you may call (623) 873-0112 if you haven’t heard back. For privacy reasons, results are not provided over the phone.
Prescriptions, Refills, and Referrals
- Refills: Contact your pharmacy at least 5 days before your medication runs out. They will fax us a refill request.
- Specialist Referrals: We aim to request specialist appointments within three business days. The specialist’s office or our staff will contact you within that timeframe. Depending on insurance pre-authorization, processing can take up to two weeks. If you haven't heard anything after two weeks, call us at (623) 873-0112.
Medical Records and Forms
- Standard forms are available at the front desk or on our website.
- Patients must provide their own forms (FMLA, record requests). Mental health professionals will not complete these forms until a patient has been established for at least two years.
- Fees: There is a $40 fee for medical records (waived if requested by another doctor for ongoing care). Please have your provider fax a signed authorization to (623) 873-1370.
Financial Policies
- Returned Checks: A $25 service charge applies to all NSF checks. Repeated issues may result in your account being moved to CASH ONLY status.
- Copays: All copays are due at registration. Check with your insurer or Arizona DES to determine if your AHCCCS plan requires a copay.
Controlled Substances
For safety and to minimize risks, the clinic follows strict narcotic prescribing rules:
- We do not prescribe controlled substances to patients unknown to the clinic.
- Refills are not provided earlier than the agreed-upon date.
- Patients must use a single doctor and a single pharmacy for these medications. Irregularities may result in prescription cancellation.
Conduct and Communication
- Contact Info: It is the patient's responsibility to keep phone numbers, addresses, and emails updated.
- Behavior: We strive to make your visit pleasant and ask that you remain respectful toward our staff. Abuse of any kind is not tolerated and may lead to dismissal from the practice. Please contact management in writing regarding any issues.
- Bilingual Service: Our clinical staff provides services in both English and Spanish.
